About the SNP Alliance

The SNP Alliance is a group of leading specialized managed care programs working together to improve quality, cost and care for high-risk/high-need persons. These managed care programs are called Special Needs Plans or SNPs. Alliance members include all major SNP types, organizational structures and regions of the US, including representation from all national demonstrations transitioning to SNP status and States with advanced statewide integration programs.

Our mission is to improve the long-term business viability of Special Needs Plans.

Our goal is to improve total quality and cost performance in serving high-risk beneficiaries.

Our work is to improve payment and health policy for high-risk care through legislative and regulatory advocacy.

The SNP Alliance pursues these goals through national leadership, regulatory and legislative advocacy and health policy research. Members actively participate in the work of the Alliance through bi-annual meetings, monthly conference calls with Alliance staff and regulatory counsel and routine conference calls with three leadership groups:

  • Policy Leadership Group
  • Medicare/Medicaid Integration Leadership Group
  • Performance Evaluation Leadership Group

The Promise of SNPs

SNPs, by design, focus on those individuals with high risk/high need profiles, such as people with multiple chronic conditions, who are low-income, who are residing in a nursing home setting, or who have complex care needs due to disability, behavioral health, or advanced care needs. SNPs represent the largest specialty care networks within managed care programs in the United States.
